PST: Injury sees Dempsey miss out again for Spurs

Last week it took us by surprised, but after getting word of his injury, concern about Clint Dempsey?s exclusion from Tottenham?s team against Stoke proved much ado about nothing. Whereas the first reaction from U.S. Men?s National Team fans was one of concern, word of ?Deuce?s? groin injury help assuage worries the USMNT linchpin was losing time with his club.

But over the Christmas holiday we heard very little about his (or anybody?s) fitness, so when the U.S. international was again excluded from Andr? Villas-Boas?s team for today?s match at Villa Park, there were renewed concerns. Only this time, the concerns were accompanied by a bit of conspiracy.

Where?s Clint Dempsey? And why don?t we know more about his status?

Today against Aston Villa, Villas-Boas has selected the same four players who started at forward and wide midfield in Spurs? 0-0 weekend draw with Stoke. Emmanuel Adebayor and Jermain Defoe were up top. Gareth Bale and Aaron Lennon wide.

And just under an hour into today?s match, the results have been much the same. That Tottenham has continued to struggle without Dempsey will provide some solace for Deuce?s biggest supporters, even if Defoe?s broken through near the hour.

Still, all this fretting exaggerates the influence of Dempsey?s injury. While it?s true that the former Cottager may see slightly less time in a fully healthy Spurs team, he?s not going to be forgotten. Villas-Boas clearly values Dempsey?s contributions, which is why the American was not only immediately inserted into Spurs? starting XI after his arrival but also maintained his place during his early season adaptation.

And paranoia about his injury status may also go too far. Given the nature of groin injuries, if Dempsey was hurt on Saturday, it?s not unreasonable that he?d continue to have problems today. It?s only been three days.

Until we hear more, there?s no reason to think Dempsey?s fitness is a serious concern. Nor is there reason to think he?s fallen out of favor at White Hart Lane.

Some cancer docs say their income tied to treatments

NEW YORK (Reuters Health) - A survey of cancer doctors finds that some believe they get paid more when they administer their patients' chemotherapy and other drugs, raising concerns about conflict of interest and the potential for overtreatment.

Researchers found that oncologists, surgeons and other cancer specialists who get paid based on the number of services they provide were seven times more likely to say their pay increases when they oversee their patients' chemotherapy treatments, compared to doctors who are paid a flat rate or salary.

"I think there is evidence out there that doctors are very responsive to financial incentives... So I think some patients should realize that doctors who are prescribing chemotherapy may be benefiting financially," said Dr. Nancy Keating from Boston's Harvard Medical School and Brigham and Women's Hospital, the study's senior author.

According to Keating and her colleagues, who published their study in the Journal of Clinical Oncology on Wednesday, some cancer doctors offer chemotherapy and other therapies in their offices.

Indeed, the researchers note, previous studies have found that as much as 65 percent of an oncologist office's income typically comes from administering treatments. The rest comes from evaluating and managing patients' cancers.

There are some advantages to patients in getting treatments at their doctors' offices, such as not having to travel to a hospital, but some observers worry the practice gives doctors incentive to prescribe more chemotherapy or more expensive drugs because they'll make more money.

In the last decade, the U.S. government tried to reduce the profit margin doctors were able to make by administering chemotherapy to their patients, but Keating told Reuters Health that didn't solve the problem.

"I think there is some evidence out there that doctors just started prescribing more profitable drugs. So this is still an issue," she said.

While the study could not determine whether patients received any unnecessary treatments, the researchers wanted to see whether doctors believe their pay is tied to their ability to give patients chemotherapy.

The researchers used a survey from 2005 through mid-2007 of 480 oncologists, who treated patients from across the country.

Overall, most participants said their income would be unaffected by prescribing chemotherapy or other drugs, or by referring patients to other specialists or hospices.

But 27 percent of the doctors said their pay would increase based on how much chemotherapy they administered and 25 percent said they'd get paid more if they administered more growth factors, such as so-called hormonal drugs, that regulate cell growth.

Doctors who were in some way paid based on how many services they provide - known as fee for service - were about seven times more likely to say their incomes would increase if they administered more chemotherapy or growth factors, compared to doctors who got a flat salary.

Doctors who had their own practices or were part of a smaller oncology group were about nine times more likely to say their income would increase if they administered their patients' chemotherapy, compared to those who worked in a hospital.

"It really is pretty substantial differences. And often, I don't think people pay that much attention to where their doctor works, but I think it does make a difference," Keating said.

But Dr. Yu-Ning Wong of the Fox Chase Cancer Center in Philadelphia cautions that the new study only looks at whether the doctors believed their pay would increase if they could administer chemotherapy or other treatments.

"It didn't say it actually did," said Wong, who wrote a commentary accompanying the study.

She also told Reuters Health that she would not want cancer patients thinking they are receiving unnecessary care or treatments based on this study.

Still, Keating and her fellow researchers say, there needs to be a new way to pay doctors "to counter or eliminate these incentives to decrease unnecessary care and ensure that health care resources are used most effectively."

Until then, Keating said patients should feel free to ask doctors whether they'll benefit financially from their treatments.

"It's certainly something I would want to know," she said. "But I agree it's not an easy thing to ask, but I do think it's something to be aware of and I think it's a fair question to ask."

Lloyd's of London insurers eye $375 million Sandy hit

LONDON (Reuters) - Three of the Lloyd's of London insurance market's leading players owned up to a combined loss of about $375 million from superstorm Sandy and warned the final bill could change.

Bermuda-based insurer Catlin , operator of Lloyd's biggest syndicate, is set to absorb about $200 million in claims from Sandy, it said on Tuesday. Rivals Hiscox and Novae put their exposure at 90 million pounds ($145.79 million) and between $25 million and $30 million respectively.

Sandy, a 1,000-mile wide storm that killed 132 as it swept through the north-eastern United States on October 29, is expected to cost insurers up to $25 billion, making it the second-costliest storm after hurricane Katrina in 2005.

Insurers and analysts have said the size of the affected region, which includes New York and other densely populated and industrialized areas, makes the final damage bill difficult to calculate.

"This estimate is subject to a considerable degree of uncertainty due to the size of the storm system, and the wide variety of damage it caused," Catlin said.

Sandy came at the end of a relatively uneventful year for natural catastrophes, in contrast with 2011, the insurance industry's second-costliest on record after earthquakes in Japan and New Zealand generated a claims bill of over $100 billion.

The comparative absence of big disaster claims during the first nine months of 2012 means the insurance industry is still expected to turn a profit for the year as a whole.

"We expect Sandy will have turned what would have been a very benign catastrophe year and a very strong earnings year back into a relatively average one," Joy Ferneyhough, an analyst at Espirito Santo bank in London, wrote in a note.

Catlin shares were down 3 percent by 4:45 a.m. ET against a 0.3 percent rise in the mid-cap FTSE 250 <.ftmc> share index, partly surrendering a 21 percent gain since the start of the year. Hiscox and Novae were 0.8 percent lower and 1.9 percent higher respectively.

Top U.S. insurers AIG and Allstate have estimated their Sandy losses at $1.3 billion and $1 billion respectively, while Swiss Re , the world's No.2 reinsurer, has said it expects to pay out $900 million.

The Lloyd's of London market, a collection of about 80 competing insurance syndicates that sell protection against natural catastrophe and other risks worldwide, has not yet said how much Sandy will cost it collectively.

Lloyd's which traces its origins back 324 years to a London coffee house where merchants met to insure ships, has historically borne about 10 percent of the claims burden from major natural disasters, analysts say.

Lloyd's insurer Beazley expects to take a $90 million hit from Sandy, it said on Friday.
